What is Breast Augmentation?

Breast augmentation is a surgical procedure involving the placement of implants under the soft breast tissue to enhance the volume and shape of breasts. It can improve your personal body image, increase your cup size, help correct unevenness in breast size or shape, and restore breasts after changes caused by childbirth, breastfeeding, losing a significant amount of weight, or aging.

How you look has a profound impact on your psyche, and without that satisfaction, you may never achieve the self-fulfillment you deserve, according to a 2007 University of Florida study led by Dr. Figueroa-Haas, women who have had breast augmentation surgery experience a boost in their self-esteem to the highest levels.

Type of Implant

There are several options for the type of implant you can have, including:

  • Silicone breast implants (filled with silicone gel) are the most popular. They are form-stable implants (often called “gummy bear implants”) that feel most similar to natural breast tissue and maintain their shape even when the implant is broken
  • Saline breast implants (filled with sterile salt water)
  • Fat transfer (fat cells are removed from other areas of your body and injected into your breasts)

It is important to remember that even today’s high-quality breast implants may only last about 15-20 years and that future surgery may be required to replace either one or both of the implants.

Incision Placement

Before the breast implant is placed in the body, your surgeon meticulously cauterizes every blood vessel in the area of insertion, eliminating any blood loss and buildup of fluid during recovery. In addition to aiding your recovery, you do not need drains after surgery, even when the incision is placed in the armpit (trans-axillary). Your surgeon does this wherever the incisions are placed, whether it is in the breast fold (infra-mammary), in the armpit, or around the areola (peri-areolar).

Bloodless breast augmentation is Dr. Edelstein’s original innovation; as such, he has developed methods to implant through all three of these major incision types.

The advanced technique is a tribute to Dr. Edelstein’s commitment to designing a safer procedure that shortens patients’ recovery times. Our surgeons use advanced techniques and tools to ensure they make the smallest incisions necessary and minimize tissue damage during surgery. An incision must be made on or beside each breast so that the implants may be inserted, but scarring will be minimal and very difficult to see once your body has been healed. 

There are three possible incision locations to insert the implants:

  1. The first is placing the implants through an incision underneath the breast, in the inframammary fold. This incision is the most popular option because it has the lowest complication rate. It gives your surgeon the best opportunity to visually plan and prepare the pockets for your implants.
  2. The second possible incision, the peri-areolar incision, is placed around the nipple’s areola. This option allows the surgeon to minimize the appearance of scarring for patients with darker skin.
  3. The third incision, a transaxillary incision, can be placed through the armpit. This incision leaves the scars on the breast itself.

Implant Placement

The Keller Funnel, also called the “No-Touch Technique,” is a single-use medical device that is designed to make the insertion of cohesive gel silicone breast implants easier due to an internal hydrophilic coating that becomes very slippery when wet. It can be used for textured or smooth implant surfaces.

The implant is passed from its sterile container directly into the Keller Funnel and then slides through it into the space created inside the breast. In this way, the implant never touches the surgeon’s gloved hands or the skin of the patient’s breast, which is known as the “No Touch Technique.”

The Keller Funnel is believed to reduce the risk of capsular contracture, although the risk of capsular contracture is already extremely low for our patients—only 1%, which is exponentially lower than the current average of 10%-25%!

Pocket Selection

Once the incision is made, space is created for the implant, called the Pocket.

There are three options for pocket placement:

  1. Under the breast and chest muscle (called Subpectoral) – this is most commonly used in women with minimal breast tissue for a more natural result
  2. Under the breast itself (Submammary and Subfascial) – recommended for women with droopiness or those who want a rounder look.

What’s the first step?

Deciding whether or not to undergo breast surgery is a completely personal decision based on balancing achievable aesthetic benefits with potential risks.

 The first step will be to schedule a consultation with one of our surgeons at Edelstein Cosmetic in Toronto, ON. During the consultation, your surgeon will be able to assess how your unique circumstances (health, physical features, expectations, etc.) can affect the success of your surgery. After evaluating your breasts and body, the two of you will review all of your implant options to pick out the type, size, and best placement for your implant.

Using all of the information collected, your surgeon will create your procedure plan, after which our Patient Coordinator can schedule your surgical date. We will provide you with a list of instructions to follow before your surgery to better ensure a safe and successful procedure. These can include not taking vitamin E, any aspirin-based medications, or garlic or fish oil tablets for two weeks before surgery. Avoid using alcohol before surgery because it can compromise your recovery.

What to Expect on the Day of Breast Augmentation Surgery

Your breast augmentation procedure will vary depending on which type of breast implants you are receiving, but regardless of which one is a part of your plan, you will begin by meeting with your surgeon to discuss your surgical plan in case you have any last-minute questions and/or concerns and have your breasts marked for the procedure in privacy. You will then come to the OR, and an anesthetist will administer your general anesthesia to provide you with a more comfortable experience. 

After you are asleep, the surgeon will make incisions hidden in your natural creases and folds to minimize visible scarring. Some incision placement options include:

  • In the breast crease (inframammary incision)
  • In the armpit (transaxillary incision)
  • Around the areola (periareolar incision)

There are a few options for where your breast implants can be inserted, but most commonly, they are placed under the pectoral muscle or directly behind the breast tissue, but over the pectoral muscle.

Once finished with the implant, your surgeon will close the incisions using layered sutures in the breast tissue to provide extra support and then close the external skin incisions with absorbable sutures.

On average, an augmentation should take about 1 to 1.5 hours without any complications.

Breast Augmentation Recovery

You will most likely want to recover from your breast augmentation surgery and return to your normal schedule as quickly as possible. The faster, the better, and the sooner you’ll be able to see your results, which is why we perform rapid recovery breast augmentation. The rapid recovery technique has allowed many of our patients to return to normal, non-strenuous activities within a short period.

We seek to make your recovery as quick as possible by doing the following:

  • We prescribe proactive medications to prepare your body before surgery even begins.
  • We minimize the amount of time you spend under anesthesia, thereby decreasing recovery issues associated with its use (e.g., nausea and/or vomiting).
  • We perform the surgery delicately and with a bloodless technique. This leads to less tissue trauma, inflammation, and bleeding, and it also reduces pain and complications, particularly capsular contracture. Less risk of capsular contracture means our patients do not routinely do post-operative massage, which can be uncomfortable.
  • We select proportionally sized implants with dimensional planning.
  • We minimize the use of medical devices (like drainage tubes).

No post-surgery massage is needed for “no massage breast augmentation.

Less risk of capsular contracture means that our patients do not routinely do post-operative massage. Massage usually starts one week after surgery, when everything is still raw inside. Massage may increase the risk of bleeding and, therefore, capsular contracture!

As a result, you’ll only need to perform a massage after your breast augmentation if it is recommended, which is very rare. We will still monitor you during the initial recovery period. If it seems that one breast is not softening normally, massage may be recommended.
